Before getting started on a new Divorce or Paternity case, we need to gather some in-depth information about our new clients. By collecting this information from you in the following documents, your attorney’s time will be used much more effectively working on your case.
We will need the following documents completed and returned to us for your attorney to begin working on your case:
Divorce WITHOUT Children:
Divorce WITH Children (There are 2 Classes):
- Divorce Questionnaire
- Vital Records Divorce Certificate
- Financial Declaration Checklist
- Financial Declaration
- Non-Public Information Forms (Complete for BOTH Parties)
Also, divorces in Utah that involve children require the parents to attend a mandatory divorce class. Besides the information in the packet, please also visit https://www.utcourts.gov/specproj/dived/ to find locations, times, and more info. It is essential that both parents take the class, though they do not have to take it together.
Paternity Order (custody without marriage):
- Paternity Questionnaire
- Non-Public Information Forms (Complete for BOTH Parties)
- Financial Declaration Checklist
- Financial Declaration
We need these documents completed and returned before your attorney will be able to work on your case. Please be as thorough as possible, but if you do not have access to a specific piece of information, just try your best to fill it out and get it back to us.
