Oddly, however, even people who doctor shop tend to fill all their prescriptions at one pharmacy. So, instead of trying to track down multiple doctors at multiple locations, it is many time easier to find that one pharmacy and get information there.

All records, documents, correspondences, doctors’ and nurses’ notes, bills, payment histories, records of all prescriptions called in, records of all prescriptions filled, dates of pickup of prescriptions, amounts of medications prescribed and filled, and any and all other records you may have in your possession for XXXX, date of birth XXXX XX, XXXX, Social Security Number XXXX.
